HACC Program for younger people (HACCPYP)

HACCPYP provides services for people with disabilities and their carers. Younger people are defined as those aged under 65 and aged under 50 for Aboriginal people.

HACCPYP provides basic support and maintenance services to help people who need assistance to remain living at home as independently as possible by providing a range of services to support them in their own home.

How can I be referred?

You may refer yourself for HACCPYP by calling Inglewood & District Health Services on (03) 54 317 000.  Referrals may also be received from doctors, hospitals, family and friends (with your consent).
