Commonwealth Home Support Program (CHSP)
The Commonwealth Home Support Program is funded by the Australian Government, and provides a range of entry-level aged care services for older people who need assistance to keep living independently at home and in their community. It is about building on your strengths and abilities to help you remain living independently and safely at home.
The program is for people aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people).
The program also includes support services for prematurely aged people on a low income who are 50 years or over (45 years or over for Aboriginal and Torres Strait Islander people) and are homeless or at risk of homelessness.
Services that can be provided:
- Domestic assistance
- Food services
- Personal care
- Home maintenance
- Social support

Care Finder
Local, Personalised and face-to-face navigation support.
What is a care finder and how can they help you or someone you know?
Care finders support vulnerable older people who would not be able to arrange services without with intensive support and do not have a family member or friends who can help.
Care finder services are provided at no cost as they are fully funded through Primary Health Networks.
A care finder can help with:
- Accessing My Aged Care, also with accessing local other supports in their community.
- If you have difficulty communicating because of language or literacy problems
- Find it difficult to understand information and make decisions
- Be reluctant to engage with aged care or government
- Finding and short-listing aged care providers in their area
